REGISTRATION
On Thursday, August 7 we will hold our In-Person Registration, from 7:30 a.m. – 1:30 p.m. in the Commons, where you will pay any outstanding fees, pick up your student’s schedule, pick up your student’s UTA/Trax pass, and get their locker assignment. To avoid lines at Registration, you may pay any outstanding fees in advance. Check the amount you owe by going to your ASPIRE account, click the payment link, or pay directly at: https://secureinstantpayments.com?Event=5860.
- 5th Graders and All NEW Students – We need a certified copy of your Birth Certificate, a copy of your Immunization Record and/or a waiver (specific to Salt Lake Arts Academy) from the Health Department.
- 7th Graders – All 7th graders need to have had their Tdap and Meningococcal Immunizations to attend school. They may have not had these yet - get them BEFORE school starts!
- Medication at School – Form included, to be signed by parents AND physicians, specifying each medication.
- Form required to be on file for ALL prescription medications, plus over-the-counter (OTC) meds such as ibuprofen or acetaminophen needed regularly. These meds must be kept locked in the office.
- Form must be updated each year.
- A few emergency prescription meds (inhalers, epi-pens) can be kept with a student (form still needed).
- Students MAY bring enough OTC medication for one day without a form. It must be kept in their locker and never shared.
MAKE-UP REGISTRATION
If you absolutely cannot join us for Registration on the 7th, you can complete Registration in the office the week of August 11 between 9am and 1pm (except Thursday, August 14 - office closed).
FEE WAIVERS
